Passport Application- Adult - renewal - please read instructions carefully

You can apply for new passport using simplified renewal form if:
- you can present your most recent passport which was not reported as lost/stolen and it was issued with at least 2 years validity
- you haven't changed your name since last passport application
- last passport was issued to you when you were 16 years old or more
- your most recent passport is still valid or has expired less than 3 years ago.
All applicants aged 18 and over must lodge their application in person.

1. Fully completed Passport Application Form (www.passports.gov.au)
2. Two photographs, one signed on the reverse side by guarantor
3. Previous Australian passport (if applicable)
4 If born in Australia, original full birth certificate
5. If born outside Australia, Certificate of Australian Citizenship (original)
6. Photo ID (e.g. foreign ID card, foreign passport, driver's licence)
7. Additional proof of identification (e.g. bank card, social insurance card, health card etc.)
8. Proof of Address (e.g.: ID with address, utility bill or bank statement with name and address)
9. If applicable, evidence of change of name (name change certificate, marriage certificate, divorce certificate etc.) with an Apostille if applicant was born outside Australia and the event happened outside of Australia.

7. Lodging parent's passport or photo ID
8. Parents' marriage certificate/death certificate, if applicable.

Emergency Passport

FOR URGENT TRAVEL ONLY.

Please provide your full name, date of birth, telephone number and email address in the comments section.

Please refer to passports' website to download the PC8 Overseas Passport Application Form and bring: two passport photos, the application form, any other ID documents you may hold. You will also be asked to demonstrate your need for urgent travel.

The emergency passport can be issued within 48 working hours once all requirements are fulfilled. This travel document will contain only 4 visa pages, has no electronic chip and the validity will be limited to a period between seven and twelve months.

It is the responsibility of the holder to ensure the travel document is suitable for travel to particular countries.
If travel is via or to a country other than Australia, please contact the respective Embassy of that country for entry and visa requirements. The Australian Embassy cannot provide visa information.

To have your driving licence renewal application witnessed by the Australian Embassy, please bring with you an application form from the Roads and Traffic Authority of the relevant Australian State or Territory, along with instructions, supporting identification and photos.

I am beyond amazed and wholeheartedly thankful for the amazing efficient thoroughness Adam presented in completing our son’s Emergency exit passport!
